Working Permit

Work Permit Application                   Physical Certification

• A parent/guardian should complete and sign PART I only of the working permit application. 
  • The student should bring in the signed application.
  • If you are not an AHHS student but you are a minor living or will be working in Elmsford, please bring your birth certificate as evidence of age. 

• All applicants must have proof of a current physical. AHHS students can check with our school nurse to see if a copy is on file.   The AT16 Physical Certification form can be completed by a doctor in place of the physical.  

• All applicants must sign the working permit card in front of the person issuing the document.

Applications should be submitted to the guidance office during normal school hours. 

During July and August, please contact the main office to submit the application.
